Financial Information
We are pleased that you have chosen our practice for your weight loss surgery! We're dedicated to providing you a comprehensive weight loss surgery program which will follow you through this life changing journey: Pre-operative consultations, surgery, as well as education and support. We understand that high quality weight loss surgery is expensive, and people often have a difficult time meeting the cost of the surgery. Therefore, we will work with you to help you find a way to pay for the surgery. This may involve paying for the operation yourself, financing the cost through a finance company, utilizing retirement funds, or working with your health insurance company. All gastric band adjustments are included for the first year.
Whether you are a self-pay patient, or using insurance, you will be asked to sign an "Advanced Beneficiary Notice". This document will state that you agree to pay the surgical professional fees and the surgery center fee one week in advance of the date of surgery. You may also be required to make an appointment with the financial specialist at Evergreen Hospital to arrange financial matters with the hospital if your surgery is scheduled there. Our office staff will send out the request for authorization for surgery.
Payment Policy
Below are three scenarios for payment
1. Weight loss surgery is covered by your insurance company & Northwest Weight Loss Surgery is a contracted provider.
In this case, you are only required to pay any co-pays or percentages as described in your policy. Co-pays are expected at the time of your first appointment. Our billing office will submit the bill to your insurance company after surgery has been completed.
2. Surgery is covered by your insurance company & Northwest Weight Loss Surgery is an out-of-network provider.
In this case, we will bill your insurance company at the time of surgery, but you will have to prepay the professional fees one week prior to surgery. As soon as we receive payment from your insurance company, you will receive a refund in the mail. Some insurance companies will cover 100% of the professional fee, some pay as little as 10%. Each insurance company is different. Please consult with your insurance company to understand your out of network benefits. The "Usual and Customary" or "Allowable Amount" may differ from the amount we bill to the insurance company.
You will be expected to pay $260 for the initial visit; this will be deducted from the professional fees if you decide to go ahead with surgery.
3. Weight loss surgery is not covered by your insurance company & you're electing to self pay.
We will collect $260 at the time of your first visit. This will be deducted from the professional fee if you decide to go ahead with surgery. The remainder of the cost of surgery will be due one week prior to your scheduled surgery date. We accept cashier's checks, money orders, and credit cards.
